Human Chorionic Gonadotropin (HCG) is given as an injection under the skin or into a muscle. HCG is a hormone that supports the normal development of an egg in a woman’s ovary and stimulates the release of the egg during ovulation. HCG helps to become pregnant.

Indications:
Inceptova HCG is used in the treatment of:
- Pre-pubertal cryptorchidism (not due to anatomical obstruction)
- Hypogonadotropic hypogonadism (hypogonadism secondary to a pituitary deficiency) in males
- Induction of ovulation and pregnancy in the anovulatory, infertile woman in whom the cause of anovulation is secondary and not due to primary ovarian failure, and who has been appropriately pretreated with human menotropins
Dose administration:
- Usual Adult Dose for Ovulation Induction: HCG: 5000–10,000 units IM once 1 day following the last dose of Menotropins.
- Usual Adult Dose for Hypogonadism – Male: 500–1000 units IM 3 times a week for 3 weeks, then 500–1000 units IM 2 times a week for 3 weeks or 4000 units IM 3 times a week for 6–9 months, then 2000 units IM 3 times a week for 3 months.
- Usual Pediatric Dose for Prepubertal Cryptorchidism: Four years and older: 4000 units IM 3 times a week for 3 weeks or 5000 units IM every other day for 4 injections or 500–1000 units IM for 15 injections over a period of 6 weeks or 500 units IM 3 times a week for 4–6 weeks; if unsuccessful, patients should be given an additional series using 1000 units starting 1 month later.
Adverse effects:
Inceptova HCG may cause the following side effects in women:
- More Common: Bloating (mild), stomach or pelvic pain
- Less Common or Rare: Abdominal or stomach pain (severe); bloating (moderate to severe); decreased amount of urine; feeling of indigestion; nausea, vomiting, or diarrhea (continuing or severe); pelvic pain (severe); shortness of breath; swelling of feet or lower legs; weight gain (rapid).
In men inceptova HCG may cause the following side effects:
Less Common: Acne; enlargement of penis and testes; growth of pubic hair and rapid increase in height
Additionally, inceptova HCG may cause these side effects in men: - Difficult or labored breathing
- Sudden, unexplained shortness of breath
- Flushing of skin
- Itching of skin
- Redness of skin
- Skin rash
- Hives or welts
- Chest pain
- Severe and sudden headache
- Slurred speech
- Sudden loss of coordination
- Severe weakness or numbness in arm or leg
- Tightness in chest
- Unusually warmth
- Vision changes
Warnings and Special precautions:
- Hypersensitivity: Anaphylaxis has been reported with urinary-derived HCG products.
- Thromboembolism: Arterial or venous thromboembolism may occur, patients with a history or family history
- of thrombosis, severe obesity or thrombophilia are at an increased risk.
Disease-Related Concerns: - Asthma: Use with caution in asthma patients.
- Cardiovascular disease: Use with caution in patients with cardiovascular disease.
- Cryptorchidism: May induce precocious puberty in children being treated for cryptorchidism. Discontinue if signs of precocious puberty occur.
- Migraine: Use with caution in patients with a history of migraines.
- Renal impairment: Use with caution in patients with renal impairment.
- Seizure disorders: Use with caution in patients with a history of seizure disorders.
Contraindications:
Inceptova HCG is contraindicated in the following conditions:
- Hypersensitivity to chorionic gonadotropin or any component of the formulation
- Precocious puberty
- Prostatic carcinoma or other androgen-dependent neoplasms.
- Prepubertal males with signs of anatomical obstruction; sex hormone-dependent tumors (e.g. ovary, breast and uterine carcinoma in females; breast carcinoma males); malformations of the sexual organs incompatible with pregnancy; fibroid tumors of the uterus incompatible with pregnancy.
Inceptova HMG is a preparation of gonadotropins, extracted from the urine of postmenopausal women which has undergone additional steps for purification. Each vial of Inceptova HMG contains 75/150 IU of FSH activity and 75/150 IU of LH activity. The biological activity of Inceptova HMG is determined using the BP bio-assays for FSH (ovarian weight gain assay in female rats) and LH (seminal vesicle weight gain assay in male rats), modified to increase the accuracy and reproducibility of these assays.

Indications
- A thorough gynecologic and endocrine evaluation, including an assessment of pelvic anatomy must be performed before treatment with Inceptova HMG. Patients with tubal obstruction should receive Inceptova HMG only if enrolled in an IVF program.
- Primary ovarian failure should be excluded by the determination of gonadotropin levels.
- Careful examination should be made to rule out the presence of an early pregnancy.
- Patients in late reproductive life have a greater predilection to endometrial carcinoma as well as a higher incidence of anovulatory disorders. A thorough diagnostic evaluation should always be performed in patients who demonstrate abnormal uterine bleeding or other signs of endometrial abnormalities before starting Inceptova – HMG therapy.
- Evaluation of the partner’s fertility potential should be included in the work up
Dose administration
Assisted Reproductive Technologies: The recommended initial dose of Inceptova HMG for patients who have received a GnRH agonist for pituitary suppression is 225 IU. Based on clinical monitoring (including serum estradiol levels and vaginal ultrasound results) subsequent dosing should be adjusted according to individual patient response. Adjustments in dose should not be made more frequently than once every two days and should not exceed 150 IU per adjustment. The maximum daily dose of Inceptova HMG given should not exceed 450 IU and dosing beyond 20 days is not recommended.
Once adequate follicular development is evident, HCG should be administered to induce final follicular maturation in preparation for oocyte retrieval. The administration of HCG must be withheld in cases where the ovaries are abnormally enlarged on the last day of therapy. This should reduce the chance of developing OHSS.

Warnings and Special precautions
- Inceptova HMG is a drug that should only be used by physicians who are thoroughly familiar with infertility problems.
- Overstimulation of the Ovary during Inceptova HMG
Ovarian Enlargement: Mild to moderate uncomplicated ovarian enlargement which may be accompanied by abdominal distension and/or abdominal pain occurs in approximately 5–10% of women treated with Menotrophin and HCG and generally regresses without treatment within 2 or 3 weeks. The lowest dose consistent with expectation of good results and careful monitoring of ovarian response can further minimize the risk of overstimulation.
If the ovaries are abnormally enlarged on the last day of Inceptova HMG therapy, HCG should not be administered in this course of treatment. This will reduce the chances of development of the OHSS.
Contraindications
Inceptova HMG is contraindicated in women who have:
- A high FSH level indicating primary ovarian failure.
- Uncontrolled thyroid and adrenal dysfunction.
- An organic intracranial lesion such as a pituitary tumor.
- Sex hormone dependent tumors of the reproductive tract and accessory organs.
- Abnormal uterine bleeding of undetermined origin.
- Ovarian cysts or enlargement not due to polycystic ovary syndrome.
- Prior hypersensitivity to menotrophin or Inceptova HMG.
- Inceptova HMG is not indicated in pregnant women.
Inceptova FSH is a product containing a highly purified preparation of human follicle stimulating hormone (hFSH) extracted from the urine of postmenopausal women. Inceptova FSH is a sterile, lyophilized product intended for intramuscular (IM) or subcutaneous (SC) injection after reconstitution with Sodium Chloride 0.9% w/v Injection provided with this pack.

Inceptova FSH provides ovarian follicular growth and maturation in females who do not have primary ovarian failure. Also stimulates spermatogenesis in males (off-label use).
Dose Administration
Adults – at first, 225 IU injected under the skin once a day. Doctor will adjust the dose after 5 days, if needed. However, the dose is usually not more than 450 IU per day. Do not use FSH and LH for more than 20 days.
Adverse Effects
Inceptova FSH may cause the following side effects:
- Back pain
- Tenderness in the breast
- Warmth
- Redness of face, neck, arms, and upper chest
- Menstrual changes
- Muscle pain
- Unusual tiredness or weakness
In rare cases, inceptova FSH may cause severe abdominal pain, bloating, chest pain, difficulty in breathing, indigestion, headache, pain or swelling in the arms or legs, and rapid weight gain.
Warnings and Special precautions
- Do not use Inceptova FSH, if you have primary ovarian failure, abnormal vaginal bleeding, uncontrolled thyroid or adrenal gland disorders, an ovarian cyst, breast cancer, uterine or ovarian cancer, a tumor of pituitary gland or hypothalamus or infertility that is not caused by lack of ovulation.

Inceptova r-FSH is a recombinant form of Urofollitropin (FSH) for Injection 75IU. r-FSH is used for stimulation of multifollicular development in ART procedures. Anovulation [including Polycystic Ovarian Disease (PCOD)] in women who have been unresponsive to treatment with Clomiphene citrate. Stimulation of multifollicular development in patients undergoing superovulation for ART such as IVF, Gamete IntraFallopian Transfer (GIFT) and Zygote Intra-Fallopian Transfer (ZIFT).
